The new “Participant Consent Form for Biobank” will be use starting today.

• In response to regulatory changes and operational needs, the Participant Consent Form for Biobank in Linkou Branch has been revised and approved by the Ministry of Health and Welfare on Nov. 29, 2024. Colleagues are kindly requested to obtain the latest version of the Participant Consent Form for Biobank (Nov. 29, 2024. version 5) signed by the participant before submitting specimens to Biobank.
• According to the regulations of competent authority, the transition period for using both the old and new consent forms is limited to one month. The old version (Mar. 5, 2020. version 4.) of the Participant Consent Form will be completely discontinued starting Dec. 29,2024.
