CGRD Data Request and Guide

Regulations for the operation and management of the Chang Gung Medical Research Database (CGRD)
Applicants are requested to read the CGRD Regulations (available for download only for internal staff on the Intranet/Institutional Regulations/Institutional Regulations/Research, please contact the Department of Medical Research of the Administrative Center for any questions or suggestions regarding the contents of the regulations).
The application data can only be used within the CGRD workstation (CGRD data will not be transferred to the ground). The Center is responsible for the operation of CGRD workstations in accordance with the regulations; in principle, the implementation of the CGRD policy follows the regulations of the Center for Health and Welfare Data Science.
Qualifications of applicants: Applicants who fulfill one of the following requirements are eligible to apply:
           (1) A full-time attending physician of this hospital or a full-time, part-time attending physician, resident, clinical researcher, administrative, medical and technical, or nursing staff of Chang Gung University.

           (2) Teachers of Chang Gung University and Chang Gung University of Science and Technology who are at the rank of assistant professor or above are required to collaborate on research projects with attending physicians specializing in related disciplines or fields in the Hospital before submitting an application.

 

Application Procedures for CGRD Data Use
Applicants are advised to search for the course code: G3S0RS230001 (the course textbook has a translation book for download) on the Chang Gung Learning Network, and for other related course codes, please refer to the Center's webpage/Chang Gung Medical Research Database/CGRD Related Courses (Chang Gung Learning Network).
Please take the above courses to understand the CGRD information and application process before you start to apply for CGRD data.
If you need to know more about the operation of the CGRD application system, what data files are included, and the year, field, and institution of the file you can apply for, please go to the intranet of the hospital/Application and Query/Research and Development/Chiang Gung Medical Research Database: Please click on the function at the bottom of the log-in page: file format and description of the whole hospital (including codebook), and the operation manual. If the page is abnormal, please contact Mr. Jiang at #6875. If you need the codebook, you can download it by logging into the internal area at the bottom right of this page and then selecting the drop-down menu of Chang Gung Medical Research Database (CGRD). (If you are unable to log in to the internal area of our website, please contact Dr. Wang at #2035).
The CGRD webpage, query system, and translation book are designed and maintained by Chang Gung Medical Center (Information Management Department). If you cannot log in, or if there is any problem with the webpage, or if you have any questions about the contents of the translation book, please contact Chang Gung Medical Center at #6873, #6874, or #6875. If you want to know more about the operation of the CGRD query system, please contact the CGRD Consultation Center for more information (NT$600/hour). For further information on CGRD data access, applicants may also make an appointment for our database counseling service ($600/hr).
Application Procedure
1. The applicant must first apply for the IRB, please note that when applying for the IRB, the applicant is required to attach the CGRD information form, which is located in the Annex of the Regulations for the Operation and Management of Chang Gung Medical Research Databases.

2. After IRB approval: Please go to our intranet/application and query/Chang Gung Medical Research Database to query the information according to the IRB approval document and the research proposal. Upon completion of the inquiry, please click on the transfer application and fill out the electronic form (Chang Gung Medical Research Database Application Form), which will be reviewed by the Medical Research Department of the hospital. For more information about the administrative process and the progress of the application review, please contact the Medical Research Department at the Lin Kou Campus #7675.

3. After the application form for use of the Chang Gung Medical Research Database has been reviewed and approved by the Medical Research Department of the hospital, the applicant's electronic form will receive a notification of the closure of this form, and then you will need to wait for another 2-4 weeks to receive the Chang Gung Medical Department's Confirmation of Changes in Procedures form and the data conditions file, which will be submitted to the departmental supervisor for closure of the case after the applicant confirms the comments and attachments in the form. (If the applicant does not receive the program change confirmation form within four weeks after the CGRD application is closed, please contact Chang Gung Medical Center #6874).

4. After the CGRD application form has been completed, the applicant will receive a notification of case closure. Within seven working days after the closure of the application form, the Center will notify the applicant by letter, and the applicant must read the instructions in the letter (which includes the case-specific appointment account number) and then go to the CGRD workstation online appointment platform to make an appointment.

5. Before entering the CGRD workstation for the first time, please read the relevant rules and regulations and the regulations on the use of the workstation.

 
Introduction of CGRD Workstation
1. Location: 3rd floor of the Research Building.

2. Open Hours: Work hours (Monday through Friday or Saturday) 09:00-13:00, 13:00-17:00, 17:00-20:00.

3. Before making the first appointment for a workstation:

After receiving the notification letter from the Center, please download the "CGRD Workstation Application Form" from the Center's website, fill out the form and return it to ccling999@gmail.com. After returning the form, you can start to reserve the CGRD workstation; those who are not listed in the IRB are not allowed to apply for access to the workstation. (If there is a change in the IRB team for the same case, you need to fill out this form again.)

4. CGRD workstations are subject to access control:

4.1 Access to the CGRD Workstation is limited to those who have been approved to use the CGRD, and the access to the CGRD Workstation must be listed in the IRB approval letter, and the period of use is limited to the period of execution approved by the IRB.

4.2 When the applicant arrives at the 3rd floor of the Research Building for the first time after booking the workstation, the administrator will ask the applicant to present his/her employee identification card and assist in setting up the access control. If the applicant is an IRB-approved staff member who is not an employee of the department and does not have an IRB ID card, the Center staff will assist the applicant in opening the door each time the applicant enters or leaves the workstation, and will not be able to provide the applicant with a separate access card.

5. CGRD workstation reservation, data carry-in and carry-out instructions: applicants and analysts are requested to read the Center's notification letter and the CGRD workstation instruction document (containing data carry-in and carry-out instructions), and to call (403-7721, 7972) if they have any questions.
