【How to Apply Medical Student Clerkship Application】
You have to apply at your specific interest specialty directly in order to inquire for an Observer position.
Once your clerkship is officially supported by the specialty (List), make sure to submit your full application documents to the secretary.
Please complete the application form and submit all required documents 2 months before the date of the period you are applying to.

2) Photocopy of enrollment certification (e.g. Student card with valid date)
3) Photocopy of achievement report (should include course details, if you choose General surgery dept., we will need to see surgery related courses on report)
4) Photocopy of passport
5) Letter of recommendation
6) non-disclosure agreement (Download)
7) Personal Curriculum Vitae and Rotation Training plan
8) Physical examination report in English version (Check List) (When your application is accepted by department, please do the examination within 3 months before training date to Chang Gung Memorial Hospital)
Also, please note that Physical examination report in English version in your serving hospital (including Hepatitis B [HBsAg, Anti-HBs antibody], Hepatitis C [Anti-HCV antibody] (within 6 months), Chest X-ray for tuberculosis, Measles IgG, Rubella IgG, RPR/VDRL (within 3 months) before coming here. If the result demonstrates that you do not have anti-HBs, you will need to be antibody vaccinated against HBV and provide proof document to us when you come to Chang Guang Hospital . If the result demonstrates that you do not have enough Measles IgG or Rubella IgG antibodies, you will need to receive MMR vaccine and provide proof document to us when you come to Chang Guang Hospital.
