Chang Gung receives the largest number of international patients in all of Taiwan, treating over 10,000 foreign patients every year. Our International Medical Center can provide fast-tracked services to patients. Our coordinators will be by your side every step of the way, starting from your appointment, all the way to your airport pickup and hotel accommodations. 1. Consultation / Make an Appointment
Prior to your arrival, please complete the "client information form" below. The information you provide will allow our International Medical Center to provide you with the most suitable team of specialists in advance, in correspondence with your medical or personal needs.
(1) Please download and complete the Client Information Form.
(2) Please also provide all related medical reports from the past 3 months, including lab or pathology reports and imaging files (X-rays, CTs, MRIs, ultrasounds, etc).
(3) Please send (1) and (2) to isc@cgmh.org.tw.

2. Wait for Our Response
Within 2 business days, a medical coordinator will respond to you by email. The medical coordinator will help pair you with a corresponding medical specialist, chosen to fit your personal needs and concerns. Here is what to expect after you have filled out the online application form:
- Please tell us your preferred appointment dates.
- Please also let us know of any special requests or needs that you may have. We will try our best to make you feel as close to home as possible.

3. Treatment Plan
After gaining knowledge of your medical background, a treatment schedule specifically tailored to your needs will be drafted by our medical team. At the same time, a detailed statement describing the treatment process and its estimated costs will be sent to you by email.
4. Appointment Confirmation, Arrival, and Accommodations
Once you have confirmed and accepted the doctor's treatment schedule, your medical coordinator will proceed to setting up, or double checking your previously made, appointment date, as well as making the necessary travel visa preparations, airport pickup, and hotel accommodations.
