Department Introduction
Vice chancellor Yung-Kuei Soong was the founder and had been actively involved in establishing Laparoscopic surgery since 1989, at the same time, assigned Dr. Chyi-long Lee for studying and introducing more advanced techniques to the department. In the year 1993, the two together succeeded in the first LAVH in Taiwan, and in 1994, completed the first case of neovaginoplasty in Taiwan. In 1995, they published the first case of laparoscopic ovarian transposition in the world.In year 1996, under the support of the formal principal of CGU, the first endsocpy/laparoscopy subspecialty was established, with Dr. Chyi-long Lee being the director,laparoscopic surgery had ever leaped to a more advanced generation with an ever expanding development. The first laparoscopy-assisted tubal reanastomosis surgery in Taiwan was performed in 1997, and later also developed laparoscopic surgery for pelvic floor reconstruction and the first laparoscopic extraperitoneal Burch colposuspension for treatment urinary incontinence in Taiwan. Chyi-Long Lee is currently the Persident of Taiwan Association for Gynecologic Endoscopy and Minimally Invasive Therapy,Dr. Chih-Feng Yen and Dr. Chin-Jung Wang also held positions in the society as the secretary and vice-secretary, they are continuing to promote endoscopic surgery in Taiwan, not only that, also help endoscopists in our department and in Taiwan to strive to a higher level world wide? Director Lee continues to be invited to many of the international seminars for endoscopy, and most recently, in close contact and interaction with Pan-Asian countries such as Singapore, Japan and Korea’s Endoscopy Society, again showing the role and importance of CGMH in the international endoscopic development and recognition. Advantages of Endoscopic surgery: A1.Small wound, less post-operative pain and fast recovery2.Under surveillance and magnification of the endoscope, the tissue and organs were greatly magnified and easily identified3.Less bleeding and post-operative adhesion Recent clinical achievementsNearly 90% of benign GYN tumor and part of the GYN oncologic disease can be safely and effectively managed with microinvasive endoscopic surgery. There are a total of 22 articles published under the first author in international and national medical journals in the years between 1999-2001. Among them, 7 were of SCI-A level, 7 B level, 1 C level and 7 in Taiwan.Conservative treatment for uterine myoma: myomectomyLaparoscopic hysterectomylaparoscopic repair of inquinal hernia 、laparoscopic repair of pelvic organ prolapse (uterine prolapse , cystocele, rectocele)and urinary incontinence surgery、laparoscopic treatment of mullerian duct anomally、laparoscopic treatment of early gynecologic malignancy Future goals and prospects1.Uterine artery ligationFor those symptoms not easily improved by the current method of surgery and for preserving the uterus in cases of uterine myoma and adenomyosis, uterine artery ligation aims to shrink the tumor size and improve the associated symptoms.2.Fetoscopic therapyWith the implementation of fetoscopic surgery, fetal movement in early pregnancy can be observed. This will help to diagnose fetal anomaly in early gestation and in reality, attempt to conduct intrauterine therapy. (ie. Therapy for TTTS) 3. Robotic surgeryIn combination with the advanced technology such as high resolution camera , specific software, and in accordance with vedio/audio system,the progression of the surgery, recording of the data, and saving of the imageries as well as telesurgery,all become possible. And under the assistance of robot, more delicate surgery is expected to be completed.4. extensive use of endoscopyin huge pelvic mass and GYN oncologic surgeriesWith specially designed surgical pocket and assistance of endoscopy,removal of huge pelvic tumor and malignant tumors no longer required laparotomy, in a way, really achieving the purpose of microinvasive surgery.
